The Eye Care Associates Rewards Program is your chance to help your friends, neighbors and co-workers save on their eye health beginning with their very first visit. You also have the opportunity to receive discounts on eye exams, glasses, and contact lenses.*

Rewards Program Discounts

Begin saving right away with these discounts:

  • Optometric Services: Eye Exams only $99.00 - You save up to $43 off our usual and customary fees ($112-$142).
  • Retinal Photos: Only $35.00 - You save $35.00 off our usual and customary fees.
  • Prescription Eyewear: 25% Off.
  • Non-Prescription Sunglasses: 10% Off.
  • Contact Lenses: 15% Off usual fees for contact lens fitting and materials (except disposable lenses). 10% Off usual fees for disposable contact lens materials (with one year supply purchase).

How Our Rewards Program Works

How To Get Started

Here's how our Patient Rewards Program works. You start by filling out the enrollment form below. In about 10 business days, we'll send you a personal Rewards Program Savings Card. You can start saving the very same day! With this card, you'll enjoy the discounts listed above.

 

Reward Referral Cards

We will also send you ten (10) Rewards Referral Cards. Give your Rewards Referral Cards to anyone outside your immediate family you think might need our services. Your friend, new neighbor or co-worker receives an instant $10 discount on their next visit if he or she is not already an Eye Care Associates patient. And, you receive credit for every new patient you refer! (Each referral card has a bar code so you get credit.) When you reach a certain number of referrals, you start receiving rewards. Please give only one $10 discount card per new patient referral you make; $10 discount Referral Cards MAY NOT be combined by the person you are referring.

Rewards Program Schedule

Refer new patients to earn free stuff:

  • 3 New Patients = Free Non-Prescription Sunglasses from a select collection of designer sunglasses (or $80 credit toward an upgrade or prescription sunglasses).
  • 5 New Patients = Free Complete Pair of Prescription Glasses or 6 Month Supply of Contact Lenses (total credit not to exceed $200 of product credit for glasses or contact lenses).
  • 10 New Patients = Free Eye Exam, Free Complete Pair of Prescription Glasses or 1 year supply of Contact Lenses, and Free Non-Rx Sunglasses (total value not to exceed $400).
  • 20 New Patients = 2 Free Eye Exams, 2 Free Complete Pair of Prescription Glasses or 1 year supply of Contact Lenses for two different prescriptions, and Free Rx Sunglasses (total value not to exceed $800).

When You Run Out of Cards

Please Note: Not every Rewards Referral Card you give away will lead to a new patient. When you run out of the first ten we send you, send us an email (to the address included with your first shipment) and we'll send you more!

 

Using Your Credits

Please note that once you have used earned Rewards Credit, your accrued total is adjusted for the amount of credit used. Example 1: You earned an $80 credit for referring 3 new patients and use it to purchase new sunglasses. You would then need to refer 5 additional new patients to receive the next level of Free Complete Pair of Prescription Glasses or 6 Month Supply of Contact Lenses (a $200 credit). Example 2: You earned a Free Complete Pair of Prescription Glasses or 6 Month Supply of Contact Lenses (a $200 credit) and only wanted to select a pair of non-Rx sunglasses from our Special Collection (for Rewards Program Members), you would then need to refer 3 more new patients to receive a Free Complete Pair of Prescription Glasses or 6 Month Supply of Contact Lenses.

*Program Rules & Important Disclaimers

  • You must be a current patient and have a recent Invoice Number to enroll. Have your most recent patient invoice handy when you enroll and look at the top-right for the six-digit invoice number. The six-digit invoice number must match the "Responsible Party" contact information and must correspond to the information you provide in the enrollment form.
  • Only one Patient Rewards Member per household is allowed.
  • Your Patient Rewards Program Card discounts may not be combined with previous purchases, insurance discounts or other promotional discounts. We will work to maximize your savings and minimize your out of pocket costs, which means we will select the discount that saves you the most.
  • You will only receive credit for new patients who present your Rewards Referral Cards. Your Rewards Referral Cards are assigned to your personal account. You will only receive credit if a new patient presents the Reward Referral card you distributed and the bar code on the reverse side is scanned. New patients are defined as never having been patients of Eye Care Associates. New patients may not come from the same physical address and may not have the same responsible party as the enrolled member.
  • Rewards Program Credits may not be exchanged or redeemed for cash equivalent. Credit may only be used for goods and services provided by Eye Care Associates, ODPA. Credit is intended to be used by the enrolled member and member must present Rewards Card to redeem Rewards credit or points. No checks or cash reimbursements will be issued for unused earned credit. Credit may only be transferred to immediate family members living in the same physical address as the enrolled member. Rewards credit may not be combined with other Rewards members credits or points. Unused credit may not be used to satisfy past due balances.
  • Your Patient Rewards Program membership is valid for one year from the date you enroll and will automatically renew for subsequent years. If you are unable to refer a minimum of one new patient during any 12 calendar month period, your membership will be considered inactive and your Rewards Program Discounts will not apply.
  • In order to keep program costs to a minimum, you must provide a valid email to enroll and all required data fields must be completed so that we may accurately credit your referrals to you. Your privacy is critically important - we do not share this information outside our organization.
  • Eye Care Associates, ODPA reserves the right to discontinue or modify this program at any time without prior notification. Any disputes regarding discounts offered will be settled by the management of Eye Care Associates at its own discretion and management decisions regarding disputes will be final.
